PRIMARY CARE PATIENTS

Upon scheduling your primary care appointment, you will be asked for your email address. 

A Patient Portal invitation will be emailed to you. Please make sure to check your spam folder and contact our office if you do not receive the invitation.

We require all primary care patients to set up a portal account and update their medical history, allergies, medications, insurance and demographic information, etc. prior to their visit. 

• Once you are logged in, select REVIEW MEDICAL RECORD.
• Clicking PATIENT ACCOUNT on the left hand side will allow you to input your demographic + insurance information. 
• Clicking REVIEW MEDICAL RECORD on the left hand side will allow you to update Medications, Medical History, Allergies, etc.
