IMPORTANT INFORMATION

All pre-op patients must have their BLOODWORK and CHEST-XRAY completed prior to their visit.

Orders for blood work and chest X-ray are in the IG Medical packet given to you by your surgeon.

If you are unable to find an appointment prior to your surgery date, please contact our office for additional assistance.

**Please make sure to bring your ID + Insurance Card as well as a medication list (including dosages) to your appointment**

***It is very important that you keep your scheduled visit or notify us 48 hours prior of any changes or cancellations. Missed visits may result in a delay of your surgery.

Due to the high demand for pre-op appointments and the importance of patients being cleared for surgery, patients will not be rescheduled after 2 missed visits.***

PRE-OP SCHEDULING INSTRUCTIONS

Please read the following in its entirety before proceeding:

Click the link below
Select your desired available appointment time on the calendar
Fill out the requested information and select continue
For "appointment type" select Annual or Physical Exam
Create a username and password **keep this handy as you will need it to log into your patient portal**

Once you have scheduled your appointment, you must log in to your patient portal, select REVIEW MEDICAL RECORD and fill out your medical / medication history. We require all patients to complete this prior to their visit.
